Step 2
Bring the following items with you to your appointment:
- Health Insurance card
- Photo ID
- Primary Care Physician Referral (if required by your insurance plan). Please make sure you obtain this before your appointment.
- Payment method. For co-payments, we accept personal checks, Visa, MasterCard, Discover, Care Credit and cash.
